Insurance & Financing

At C3 Wellness Spa, we accept major medical insurance plans and VA benefits for eligible services including massage therapy, physical therapy, and acupuncture. To facilitate accurate benefit verification and claims processing, patients must present a valid insurance card and government-issued photo identification at the time of service. For veteran patients, please coordinate with your VA physician or care coordinator to request authorization for treatment at C3 Wellness Spa. Once authorized, a member of our team will contact you directly to schedule your appointments.

Please note: Insurance participation is evaluated on a per-policy basis. While we may be listed under certain insurance networks, coverage and in-network status are not guaranteed for all plans. Our staff is available to verify benefits and provide clarification regarding your specific coverage.

• Aetna (PPO, Indemnity, Medicare Plans)
• AARP (Medicare Advantage Plans, Supplement Plans)
• BlueCross Blue Shield (BCBS Local & Out of State Plans, some FEP Plans)
• CarePlus (requires prior authorization from referring physician)
• ChampVA
• Cigna only as a secondary (excludes Commercial Plans/ASH, Disney Plans, and HMOs)
• Emblem Health (Empire BCBS, Empire UHC Secondary ONLY)
• Empire Plan
• GEHA
• Humana (some policies require prior authorization from health plan)
• Medicare
• Tricare (For Life or Select)
• United Healthcare
• VA Community Care
• WellCare (some WellCare plans require prior authorization from referring physician)

Other insurance will be accepted on a case-by-case basis after policy verification. Insurance verification of benefits will be required prior to the appointment to confirm copays, deductibles, and coinsurance.
